Feldenkrais® for Upper Back Pain: What Fits When

Upper back pain often grows from long sitting and a held ribcage. Here is where slow Feldenkrais lessons fit, and where Pilates, yoga or strength work fit better.

In short

For upper back pain that comes from long sitting and a held ribcage, Feldenkrais® for upper back pain tends to fit best when your back feels stiff and guarded and effortful exercise irritates it. Pilates, yoga or strength work fit better once your back tolerates effort and you want to build capacity.

Before you begin. Upper back pain with chest pain, shortness of breath, fever, unexplained weight loss, a fall or injury, or pain that wakes you at night needs a doctor before any self directed movement.

Feldenkrais® for upper back pain is a gentle option for a back that has grown stiff and guarded after years of sitting. This page compares it, plainly, with the other serious choices. It looks at the upper back, also called the thoracic area, between your shoulder blades and behind your ribs. That is a different story from the lower back.

Why the upper back gets stuck

The thoracic spine is built to move. Each vertebra connects to a pair of ribs, and together they turn, bend and move with every breath. Long sitting cuts that variety down. The ribcage settles into one shape, the shoulder blades stay parked, and the muscles between them hold on all day. Nothing is broken. The area has just forgotten some of its options.

Pain here has many possible sources, so a doctor's opinion is worth having if it is new or unusual. Our guide to why your upper back is so tight explains the usual pattern. Our Feldypedia entry on desk posture and chronic neck pain shows how the upper back and neck share it.

What a Feldenkrais lesson offers an upper back

A Feldenkrais Method® lesson, in the Awareness Through Movement® format, is done lying down while a voice guides you. You explore tiny movements of the ribs, shoulder blades, head and pelvis. Nothing is pushed or held. You notice how your ribs meet the floor, then how they shift when you turn your head or reach an arm.

The aim is not to pull your back into a better shape. It is to rediscover easy turning and breathing, so your upper back stops doing all the work alone. Many people say their shoulders feel wider and their breath feels fuller after a lesson. That is a personal experience, not a promised result.

How the options sit side by side

ApproachStrengthKeep in mind
Feldenkrais lessonsSmall, easy movements that bring the ribs and shoulder blades back into useEvidence is emerging and general, with no large trial in upper back pain
PilatesBuilds trunk strength and control with repeated exercisesEffort based, so a guarded back may find early classes a lot
YogaBuilds range and a daily habit through held shapes and stretchHeld end range shapes can irritate a sensitive mid back
Strength trainingBuilds capacity in the upper back and shouldersNeeds a starting load your back tolerates
PhysiotherapyClinical assessment and a plan for your specific backThis is clinical care. Every movement method here sits alongside it

The research on Feldenkrais is promising but modest. One review pooled 20 randomized trials of the method. Its authors saw promising signs for things like balance, noted a high risk of bias, and found no adverse events (Hillier and Worley, 2015). It says little about upper back pain in particular, so we do not claim Feldenkrais beats anything on this list.

When each option fits

Feldenkrais tends to fit when your upper back feels stiff, held or easily irritated, and you want a daily practice that never asks you to push. Pilates and strength work fit better when you already move without a flare and want to build capacity. Yoga fits if you enjoy stretch and move comfortably between floor and standing. These methods work through a different mechanism, so many people mix them.

Clinical care is a separate lane. If you see a physiotherapist or doctor, keep going. Feldenkrais lessons sit alongside that work. For a movement based starting point, see our exercises for the upper back.

Feldy was built for this group: people whose backs feel stuck from sitting and who want short lessons they can do lying on a bed or the floor. You can try it with a free 7 day trial and see how your back responds.

Find where to start

If you cannot tell whether your back wants calm or effort first, a short quiz can save weeks of guessing. It looks at how you like to move, not only where it hurts.

Unsure whether to start gently or with effort? Try the 60-second Movement Match quiz and see which movement method suits how your upper back likes to move.

Common Questions about Feldenkrais for upper back pain (FAQ)

Is Feldenkrais safe for upper back pain, and who should avoid it?

The lessons are slow, small and kept inside comfort, so they are among the gentlest options. Skip them and see a doctor first if your pain comes with chest pain, shortness of breath, fever, unexplained weight loss, or began after a fall or injury.

How often should I do Feldenkrais for my upper back?

Ten to twenty minutes most days works better than one long session a week. Short, regular practice gives your back repeated, easy experiences of moving, which is what the method relies on.

How long until I notice a change in my mid back?

Many people feel a softer, easier ribcage right after one lesson. Steadier changes in how you sit and breathe usually take a few weeks of regular practice. Pain that has lasted months rarely changes overnight.

How is Feldenkrais different from Pilates, yoga or strength training?

Pilates, yoga and strength training work through a different mechanism: effort, holding and stretch to build strength or range. Feldenkrais uses small, easy movements with attention, so it asks for much less effort. Many people use one alongside the other.

How is Feldenkrais different from physiotherapy?

Physiotherapy is clinical care, with assessment, diagnosis and a plan for your back. Feldenkrais lessons are movement education and sit alongside that care as a daily home habit. They do not replace it.

What is the best exercise for upper back pain from sitting?

There is no single best exercise. Breaking up long sitting often matters most. After that, many people like slow rotation of the ribcage and gentle strength work for the upper back. Pick the one your back tolerates and that you will keep doing.

What should I avoid with upper back pain?

Avoid forcing through sharp pain, holding one position for hours, and pulling your shoulders back hard to look upright. Heavy end range twists on a guarded back can also stir it up. Stay in a range where everything feels easy.

When should I see a professional about upper back pain?

See a doctor if the pain comes with chest pain, shortness of breath, fever, weight loss you cannot explain, or numbness or weakness in the arms or legs. The same goes for pain after a fall or pain that wakes you at night. If it has simply lasted for months and has never been checked, a visit is still sensible.
