Feldenkrais® for knee pain starts from an unpopular observation: the knee is a simple hinge stuck between two joints that can do almost anything. The hip above it rotates in every direction, the ankle and foot below it adapt to any surface, and the knee has to absorb whatever those two decide not to handle. Working on the knee alone leaves the two decision makers untouched.

The scale of the problem explains why so much is written about it. Osteoarthritis affects around 528 million people worldwide, and the knee is the most affected joint at 365 million (WHO, 2023).

Where the lesson puts its attention

The Feldenkrais Method® is interested in how a movement gets shared out: who joins in, who holds still, and what happens first. For a knee that means the hip that stopped rotating, the foot that stopped adapting, the pelvis that no longer turns as you walk, and the thigh that grips continuously in anticipation of pain.

Lessons are done lying down or sitting, in small ranges, with the leg carrying no weight. That sounds indirect, and it is. The reasoning is that a knee under too much load is often being asked to do a hip's job, and no amount of local work changes that arrangement.

What it does not do

It does not build quadriceps strength, and strengthening remains the exercise intervention with the strongest support for knee osteoarthritis. It does not change cartilage. It does not diagnose. If your knee swells, locks, or gives way, this page is the wrong page and a clinician is the right one.

Saying that plainly is not modesty. A knee that hurts usually needs two or three different things, and the common mistake is picking one and expecting it to be sufficient.

Why the coordination layer earns its place

The knee is rarely the decision maker. Hip rotation and foot adaptation determine what arrives at the knee, and neither is addressed by knee exercises.

Gripping is expensive. A thigh that never fully lets go compresses the joint all day, and that habit does not stop because the muscle got stronger.

It works on painful days. Lying down with no load means there is something available on the days when your strength programme is out of reach.

When something else fits better

If your knee is swollen, hot, locking, giving way, or was hurt in a fall, see a clinician first and leave everything else until you know what you are dealing with. If you have knee osteoarthritis and are not doing progressive strength work, start there, because it has the best evidence and a lesson will not substitute for it. If land based movement is too painful to begin, water takes the load off while you build tolerance. If standing for an hour is comfortable and you would rather practise with other people, tai chi offers that. What is the best exercise for knee pain?

For a knee irritated by a sudden change in activity, a temporary reduction in load usually matters more than any exercise. Coordination work addresses a third thing, how the leg is being used, and it fits alongside the other two rather than replacing them.

What exercises should I avoid with knee pain?

Avoid deep squats and lunges past the point where the pain sharpens, avoid kneeling on a sore kneecap, and avoid stair repeats as a training method while the joint is irritable. Avoid holding a long stretch at the very end of the bend. Avoid pushing through pain that lingers into the following morning, which is the clearest sign the dose was wrong. And avoid stopping walking altogether, since knees generally do worse with rest than with a sensible amount of movement.

Can lessons help if I already have knee arthritis?

Yes, though not by changing the joint surface, which no movement can do. Where they help is in the surrounding effort: an arthritic knee is usually surrounded by a gripping thigh and a hip that has stopped contributing, and that combination costs the joint more than it needs to spend. Many people find walking easier when the hip starts doing its share again. Keep your strength work going alongside.

How often should I practise?

Fifteen or twenty minutes on most days works better than a single long session, because coordination shifts through frequent low effort repetition. It sits easily around a strength programme, since it adds no fatigue. If a lesson leaves the knee more sore that evening, the movements were bigger than they needed to be.

How long until my knee feels different?

People often notice something within the first lesson, usually that standing up or the first few steps cost less effort, and that early impression tends to fade the same day. A difference you can rely on when walking or on stairs generally takes several weeks of short regular practice. Strength work runs on a similar timescale and gives you something different.

How is this different from physiotherapy or Pilates?

Physiotherapy assesses your particular knee, gives you a diagnosis, and loads the tissue in a progressive plan, and it is clinical care that gentle movement runs alongside rather than replaces. Pilates trains control and capacity against resistance. A Feldenkrais lesson removes load and works on the sequencing of hip, knee, and foot.
