Pain in Upper Right Back: Muscle or Something Inside?
Pain in upper right back most often comes from muscles and joints that work harder on that side. Here is how to notice your pattern, and which signs need a clinician.
In short
Pain in upper right back most often comes from muscles and joints that do more work on the right, such as mouse use, driving or a shoulder bag. Pain after fatty meals or in bouts under the ribs needs a doctor. With fever, yellow skin, chest pressure or breathlessness, call emergency services.

Before you begin. This page is general information and never a treatment. Pain in the upper right back usually comes from muscles and joints, but it can also come from the gallbladder, liver, lungs or heart, so please see your doctor about pain that is new, strong or lasting. Call emergency services for severe pain under the right ribs, pain there with fever, vomiting or yellow skin or eyes, chest pressure, pain spreading to the arm, neck or jaw, sudden tearing pain, sudden weakness, or trouble breathing. Nothing here replaces a medical examination.
Pain in upper right back is common, and most of the time it comes from muscles and joints that work harder on the right side of the body. Sometimes, though, the message comes from something deeper. This page helps you notice which kind you may have, and which signs mean you should talk to a clinician. If the pain comes with chest pressure, trouble breathing or sudden severe pain, phone emergency services before you read on. This page is information that sits alongside medical care, not a diagnosis.
The WHO fact sheet gives no figure for the upper back alone. Its nearest number is neck pain, at around 222 million people worldwide (WHO, 2022), and that count does not include the upper back. The two areas do share several muscles, so an ache here is nothing unusual.
Why pain in the upper right back is often muscular
The upper back is a busy place. The ribs meet the spine in small joints, and the shoulder blade glides over the ribs, held in place by muscles. For many people, the right side simply does more.
- The mouse hand. The right arm may reach forward for hours, and the muscles that steady the shoulder keep working the whole time.
- The dominant arm. Writing, reaching, lifting and carrying often go to the right first.
- A shoulder bag. Carrying the strap on the right means the muscles around that shoulder keep gripping for hours.
- Driving. One hand on the wheel and a turned head to check the mirror can load one side more.
- Reaching and twisting. A sudden reach behind the seat can leave a sore patch for days.
None of this needs blame. It is usually a body doing one job for a long time. If your pain sits mainly behind the shoulder blade, our page on behind right shoulder blade pain covers that spot in detail. This page looks at the wider area, and at how to tell when it is not the back at all.
Signs that pain in the upper right back may not be muscular
Pain can be referred, which means the body shows it somewhere other than its source. Under the right ribs sit the gallbladder and the liver. When these are irritated, the pain is sometimes felt in the upper right back. This is not a rule, and it is not the usual cause. It is a pattern worth knowing.
Please ask a doctor about any of these:
- pain that arrives after a heavy or fatty meal, or comes in bouts
- an ache that stays under the right ribs and spreads toward the back
- fever or chills, or dark urine
- a band of pain on one side of the body, followed by a rash of small blisters, which can be shingles
- pain that is worse with a cough or a deep breath (if you are also short of breath, treat it as an emergency)
- pain that keeps getting stronger over days
- pain that follows a fall or a blow, wakes you at night, or comes with weight loss you cannot explain
A strained muscle often changes when you move, press on it or shift position. That is only a clue, not proof. Pain that stays the same whatever you do, or arrives with the signs above, is the kind to have checked.
When right back pain is an emergency
Treat these as urgent and phone emergency services right away:
- severe pain under the right ribs, or pain there together with fever, vomiting, or yellow skin or yellow whites of the eyes
- pressure, squeezing or tightness in the chest, or pain spreading to the arm, neck or jaw
- a cold sweat, nausea or sudden faintness with the pain
- sudden, severe, tearing pain in the back or chest
- sudden weakness, numbness, trouble speaking or seeing, or not being able to control your bladder or bowels
- trouble breathing
Heart problems are usually pictured on the left, but they can also be felt on the right, in the back, the jaw or the arm. If you are unsure, treat it as urgent. It is always fine to be told it was nothing.
How to notice your own pattern with pain in the upper back on the right side
Once a doctor has no concern, or has told you the ache is muscular, curiosity can be a useful tool. Pain often has a pattern, and the pattern is easier to see on paper than in memory.
For two days, keep a short note with three parts:
- When it comes on. Morning, after sitting, after a drive, at night?
- What you ate. Especially before a flare, and whether a heavy or fatty meal came first.
- What your right arm was doing. Mouse, phone, bag, steering wheel, reaching?
Add a line on what changes it: a walk, a warm shower, a different chair. You are not looking for a verdict. You are collecting facts that you, or your clinician, can read together. A note that links the pain to meals tells a different story from one that links it to the mouse, and your doctor will want to hear either.
Small, easy experiments can sit alongside the note. Shift your chair every half hour. Notice where your right shoulder rests when you click. Bring the mouse a little closer. Let your breath widen the back of your ribs on the right, softly. Stop if anything sharpens. For a longer guided sequence, see Feldenkrais® for neck tension.
How the Feldenkrais Method® looks at one sided upper back pain
In the Feldenkrais Method®, the right upper back is not seen alone. The shoulder blade rests on the ribs, the ribs move with every breath, and the neck and arm share the work. When one part holds, the others tend to take more. Small, slow movements let you sense where you hold and where you move with ease. Many people notice that the right side feels different once they pay attention to it.
This is not a treatment, and it does not tell you what is causing your pain. It is a way of getting to know how you use yourself, alongside whatever care you are given. Feldy offers short guided lessons in that spirit. For the role of repeated hand and arm work, the Feldypedia article on repetitive strain from keyboard use is a good next read. You can also compare with upper back pain on the left, or read the wider causes of upper back pain. More lessons for this area are on our neck and upper back page.
The takeaway is short: keep your two day note, and let any warning sign send you for help before anything else.

Common Questions about pain in the upper right back (FAQ)
When is pain in the upper right back serious?
Call emergency services for severe pain under the right ribs, pain there with fever, vomiting or yellow skin or eyes, chest pressure, pain spreading to the arm, neck or jaw, sudden tearing pain, sudden weakness or trouble breathing. See a doctor soon for dark urine, a one sided band of pain with a rash, or pain that keeps getting stronger.
Can the gallbladder cause pain in the upper right back?
It can. Gallbladder pain is sometimes felt near the right shoulder blade. It can follow a heavy or fatty meal, or come in bouts under the right ribs. This is a pattern worth mentioning to a doctor, not a rule. Many people with upper right back pain have a muscular cause.
Can a heart problem show up on the right side?
Yes. Heart trouble is usually pictured on the left, but it can also be felt on the right, in the back, jaw or arm. Pressure in the chest, sweating, breathlessness or sudden faintness call for emergency services at once, whichever side hurts.
How long does a muscular ache in the upper right back last?
It varies. Many people find a sore patch eases over days to a few weeks once the load on it changes. If it lasts longer, keeps returning or grows stronger, ask a doctor to look at it instead of waiting.
How often should I move if my upper right back hurts?
Little and often tends to suit this area. A short pause every hour or so, to change position and sense your right arm, usually asks less of it than one long session. Stay within comfort, and stop if the pain sharpens or spreads.
How is right sided pain different from pain on the left?
Muscle strain can show up on either side. The right side is more often tied to the dominant hand, while organs such as the gallbladder and liver sit on the right, so a doctor takes one sided pain with other signs seriously.
Should I see a physiotherapist or a doctor first?
If you have any warning sign from this page, start with a doctor. If the pain looks muscular and you have been cleared, a physiotherapist can assess how you move. Gentle awareness work such as Feldenkrais sits alongside that care and does not replace it.
