With fibromyalgia, change often shows up beyond the pain score

In four fibromyalgia studies, the clearest change came in daily function and feeling better overall. The pain score moved least, though the evidence is early.

Your pain is a six. It has been a six for weeks. Somewhere in those weeks, though, the afternoons got easier.

A zero to ten score has no box for that. Across four fibromyalgia studies, much of the change showed up outside the pain number: in daily function, in how people felt overall, in how far the pain spread. The pain score may be the slowest number to move, so the others are worth watching too.

Watch more than pain with fibromyalgia: track the list of symptoms like light, sound and sleep; notice whether you feel better overall; notice where the pain spreads; and do less on hard days, because change may show up beyond the pain score.

The list said more

In Kutahya, Turkiye, researchers asked 65 people with fibromyalgia to name every way their body overreacted: pressure, sound, light, touch, fatigue, broken sleep. That list is the widespread sensitivity of this condition.

The longer the list, the more severe the fibromyalgia tended to be. Longer lists also went with lower quality of life and more catastrophising, which means expecting the worst from pain. Against pain intensity, the list said almost nothing.

A larger team at Dell Medical School and Ohio State gave a scored symptom questionnaire to 820 adults. Higher scores went with worse functioning, less energy and poorer general health. Pain still mattered there too. Both studies were cross sectional: single snapshots that can't show what came first. Together, they make a case for looking past the single pain score.

Feeling better versus hurting less

The drug research shows a similar gap. A McMaster University team pooled 151 trials of fibromyalgia medicines in 32,592 people. No drug's average effect on pain intensity reached the size the team called clinically important.

Most of the benefit landed on a scale of how people feel now compared with before. It folds pain, mood, sleep and function into one judgement. So a person can feel better in general while the pain score stays where it was.

With some drugs, one or two more people in ten saw their pain fall by at least 30%. All of it was graded low certainty.

Function and spread pulled ahead

In a small trial in eastern Turkiye, 40 women had either connective tissue massage or a foam roller, twice a week for six weeks.

Both groups improved on every measure, pain included. The massage group pulled ahead on everyday function, total fibromyalgia impact and how widely pain spread on a body map. The summary claimed no edge on pain intensity. There was no group without sessions, so attention and routine may explain part of it.

Six months, less on hard days

The longest study, in Greece, split 68 adults between six months of live online classes, three a week, and their usual routine. When someone reported pain or fatigue, the instructor cut the work.

A questionnaire on how much fibromyalgia gets in the way of daily life fell from about 59 to about 51 in the class group. The other group stayed near 62. Every result was self reported.

From the inside

None of these studies tested the Feldenkrais Method®, so I hold them lightly. What I recognise is the order in which change tends to arrive.

People often tell me the pain is about the same. Then, almost in passing, they mention that the stairs felt easier, or that they got through the shopping and didn't need to lie down after. From the inside, change may come first as pain that takes up less of the day.

In a Feldenkrais® lesson the movements stay small and slow, and you only go where it feels easy, pleasant, and comfortable. Less effort leaves more room to notice. If you live with fibromyalgia and want a guided option, here is how we looked at the best exercise app for fibromyalgia.

The next time someone asks you for a number between zero and ten, what else would you want to tell them?
